Abstract | 「要旨」99mTc-ECDの2分割投与と静脈採血法により, Diamox 負荷前後の脳血流をマイクロスフェアモデルに基づいて算出する脳血流測定法を考案した. 本法では入力関数の積分値を既報の1点静脈採血法により計算する. 脳血管障害など5例を対象に, 適宜に2分割した99mTc-ECDを Diamox 負荷前後で静注し, 負荷前後のSPECT撮像と静脈採血により安静時と Diamox 負荷時の脳血流量を算出した. 大脳皮質における安静時の脳血流平均値は0.47±0.07ml/g/minで, Diamox 負荷により0.63±0.12ml/g/minと増加し, 133Xe吸入法で求めた脳血流量とよく相関した. 平均増加率は35%と有意な脳血流の増加がみられた. 本法では, 動脈採血や大動脈の時間放射能曲線の測定および投与量補正が不要であり, 大視野ガンマカメラのみならず頭部専用SPECT装置や従来の小視野ガンマカメラにも適用できる. 非侵襲的で簡便であり, 臨床に有用である. |

Abstract | The purpose of the study is to develop a simple and less invasive method for quantifying regional cerebral blood flow (rCBF) at pre- and post Diamox test using split-dose 99mTc-ECD and SPECT. By employing a microsphere model, integral of input function was calculated by the one-point venous sampling method previously reported. The study was performed on 5 subjects with cerebrovascular diseases. A split dose of 99mTc-ECD was injected pre- and post Diamox injection, and rCBF was measured by two SPECT scans and single venous samples, respectively. Mean CBF obtained by the present method was 0.47 +- 0.07 ml/g/min at the control state, and 0.63 +- 0.12 ml/g/min loaded with Diamox (mean % increase; 35%), showing good agreement with those obtained by the 133Xe-inhalation method. Since the present method does not require arterial blood sampling, dynamic data acquisition and dose corrections, it is simple, less invasive and useful in clinical SPECT studies. |
