| Abstract | 「要旨」白血球シンチグラフィは感染巣および炎症巣の診断とその局在の検索に確立された検査法であり, そのsensitivityおよびspecificityは90%に達している. 近年においては炎症性腸疾患に対する適応も確立されたが, 99mTc標識白血球検査法においては至適検査時間が論議されている. さらに白血球シンチグラフィでは骨髄が明瞭に描画されることから骨髄シンチグラフィとしてのポテンシャルも検討され始めている. 炎症の定義と分類, 病態, 炎症シンチグラフィの歴史, 検査法, 診断率, および診断上の注意点を中心に概説した. 「1. はじめに」白血球シンチグラフィの歴史は1959年にさかのぼるが, 111Inで標識され, 臨床応用が可能な報告がされたのは, 1976年ThakerやSegalらによる. さらに, 1986年にPetersらにより, 99mTc標識による, 白血球シンチグラフィが報告され, 欧米においては67Gaシンチグラフィに代わる診断精度の高い炎症巣の臨床診断法として日常検査に利用されている. |

| Abstract | 111In or 99mTc-HMPAO labeled leukocyte scintigraphy has been shown to be useful in detection and diagnosis of infections and inflammatory lesions. Results from multiple retrospective series indicate that leukocyte scintigraphy achieves sensitivity and specificity of near 90% in abdominal abscess detection. 99mTc-HMPAO labeled leukocyte scintigraphy is now an accepted technique in the assessment of patients with inflammatory bowel disease and the diagnostic value of early 99mTc-HMPAO labeled leukocyte images has been discussed. It is known that 99mTc-HMPAO labeled leukocytes migrate to a large extent to the bone marrow. 99mTc-HMPAO labeled leukocytes may prove to be a practical agent for bone marrow imaging. In this report, definition, classification and patho-logical consideration of inflammation, and history, diagnostic efficacy and consideration for clinical application of inflammation scintigraphy are reviewed. |
