Abstract | 「要旨」経皮的胆管ドレナージ (PTCD) により減黄効果が得られた膵癌5例, 胆道癌9例を対象に, 胆管閉塞および随伴する胆管炎の血清SPan-1値に及ぼす影響について検討した. また, 胆道癌の4例では胆汁中のSPan-1値も併せて測定し検討を加えた. PTCD後, 膵癌1例 (20%) と胆道癌8例 (89%) で血清SPan-1値は下降した. PTCD後, 血清SPan-1値は血清CA19-9値と同様な変動を示し, また胆汁中においても両者は同様な変動を示した. このことから, 血清SPan-1値は胆管閉塞および随伴する胆管炎によりCA19-9と同様な機序で影響を受け, その影響はCA19-9と同程度であると思われた. また, 膵癌症例は胆道癌症例に比べ同じStage IVにおいても血清Span-1値が胆管閉塞および随伴する胆管炎から受ける影響は小さかった. この原因として, 膵癌が胆道癌に比べ脈管への侵襲が強い点や胆道系因子の他に膵管系因子も加わる点等が考えられた. |

Abstract | [Summary] We investigated the effect of biliary obstruction and choiangitis on serum SP an-1 level in 14 patient (5 patients with pancreatic cancer and 9 patients with biliary tract cancer) who were performed PTCD for biliary obstruction. 1 out of 5 cases with pacreatic cancer and 8 out of 9 cases with biliary tract cancer showed a decrease of SPan-1 level with improvement of jaundice. It suggested that the effect of biliary obstruction and cholangitis on serum SPan-1 level was much more in biliary tract cancer than in pancreatic cancer. There was no significant difference in changes of between serum SPan-1 level and serum CA 19-9 level after PTCD. We conclude that the effect of biliary obstruction and chorangitis on serum SPan-1 level is almost same on serum CA 19-9 level. |
