Abstract | 「1. はじめに」甲状腺機能亢進症の患者に対し, 131Iによる放射線治療が広く行なわれるようになっているが, 最近副作用として131I投与後の甲状腺機能低下症の発生が重要な問題となっている. この副作用を防ぎ適正な131I投与量を決定するための因子として, 正確な甲状腺重量の測定が必要と考えられる. 1952年すでにAllenとGoodwinは甲状腺スキャンから甲状腺重量を算出する試みをおこなっている. すなわち甲状腺重量=スキヤン面積×左右最大長径の平均×0.32なる実験式を10例の甲状腺機能亢進症の患者に適用した結果, 同式による算出重量と剖検及び手術により確かめられた真の重量との誤差の平均は僅か10.6%であったと報告している. しかしその後Kellyは15例の甲状腺機能亢進症について本式の追試を行なった結果, 平均誤差は49%であったと批判的な意見を述べている. |

Abstract | In order to evaluate the reliability of Allen's empirical formula, retrospective study was undertaken. The observed weight of the thyroid glands removed at surgery was compared with the weight calculated from scintiscan (20% cut off) of the thyroid gland using Allen's formula. The calculated weights were generally greater than the observed weights and average error was 63 per cent in our 29 cases. The mean heights of the both lobes multiplied by surface area on the scan were plotted as a function of the observed weights of the thyroid gland. In this graph, there was a suggestion of a simple mathematical relationship, so it was possible to deduce an equation which would yield the reliable weight estimated from the following equation: w=0.28 L・S-12, L・S>43, where w denoted the observed weight of the thyroid gland and L the mean height of both lobes and S the surface area on the thyroid scan. Average error was 40 per cent in our series. |
