Abstract | 「要旨」 大動脈弁閉鎖不全症や僧帽弁閉鎖不全症に対しT1-201心筋シンチグラフィを施行し, 容量負荷による左室拡張性肥大について検討した. T1-201心筋シンチグラフィのT1欠損重症度と心電図上のT波の変化, NYHAとの間に良好な相関関係が認められた. T1欠損像の増強にともないLVDdは増大し, 中等度以上のT1欠損像を呈した症例では18例中17例でLVDdは65mm以上であった. 負荷T1-201心筋SPECTより算出した左室全周のWashout rateとLVDdとの間に有意の負の相関関係が認められた (r = -0.603, p < 0.01). 負荷T1-201心筋シンチグラフィにて評価した再分布はかかる症例の経過中比較的早期より出現した. 左室拡張性肥大に伴う左室壁障害の原因として, 機械的容量負荷や心筋虚血性変化などの関与が示唆された. 中等度以上のT1欠損像の認められた症例では, 心不全, 不整脈の有無にかかわらず弁置換術を検討する必要があると考えられた. |

Abstract | [Summary] In order to elucidate the mechanism of left ventricular eccentric hypertrophy in conditions of volume overload, T1-201 myocardial scintigraphy was performed patients with aortic valve regurgitation and mitral valve regurgitation. There was a good relationship between the severity of T1-defects, as determined by T1-201 myocardial scintigraphy, and the changes in the T wave on the ECG on the one hand and the NYHA functional classification of heart diseases. In 17 of 18 patients where LVDd increased with increasing severity of T1-defects and the defects were moderate to severe, LVDd was 65mm or larger. There was a significant negative correlation between the washout rate for the whole circumference of the left ventricle, as determined by exercise T1-201 SPECT, and LVDd (r =-0.603, p < 0.01). The phenomenon of redistribution as determined by exercise T1-201 myocardial scintigraphy was observed relatively early. Our results suggest that mechanical volume overload and ischemic changes are involved in left ventricular wall damage in left ventricular eccentric hypertrophy. For patients with moderate to severe T1-defects valve replacement is indicated, no matter whether they may have heart failure or arrhythmia. |
