Abstract | 「要旨」 PTCA, CABG後のhibernating myocardiumの局所壁運動改善と心筋脂肪酸代謝との関連を検討した. 冠動脈疾患の34例に対し, 術前と術後3, 4か月に安静時201Tl (Tl) および123I-β-methyliodophenyl-pentadecanoic acid (BMIPP) の2核種同時収集心筋SPECTおよび左室造影を施行した. Isotope集積を定性的 (視覚的), 定量的 (%uptake)に, 壁運動を定性的 (視覚的), 定量的 (shortening fraction) で評価した. Hibernating myocardiumと考えられる78区域において, 壁運動改善はTl集積改善 (r=0.39) よりBMIPP集積改善 (r=0.63) に, より正相関 (p<0.05) を認めた. また, 両者の解離改善度と弱い正相関 (r=0.36) を認めた. Hibernating myocardiumの冠灌流の改善3, 4か月後の壁運動改善は心筋血流改善以上に心筋脂肪酸代謝の改善と関連した. |

Abstract | To evaluate the regional wall motion and the myocardial fatty acid metabolism at hibernating myocardium after revascularization (PTCA or CABG), we performed dual SPECT with 201Tl and 123I-β-methyliodophenyl-pentadecanoic acid (BMIPP), and left ventriculography (LVG) in 34 patients with coronary artery disease before and 3 to 4 months after revascularization. In the SPECT, regional tracer uptake was estimated qualitatively (visual) and quantitatively (%uptake). Regional wall motion was estimated qualitatively (visual) and quantitatively (shortening fraction). At the 78 hibernating areas, the improvement of regional wall motion was more significantly (p<0.05) correlated with that of regional tracer uptake of 123I-BMIPP (r=0.63) than 201Tl (r=0.39), and also correlated with the improvement of the difference between 201Tl and 123I-BMIPP regional uptake (r=0.36). These results suggest that the improvement of wall motion at hibernating myocardium is more significantly correlated with the improvement of 123I-BMIPP than 201Tl uptake after revascularization. |
