Abstract「要旨」中枢性ベンゾジアゼピン受容体(BZR)に特異的に結合する123I-イオマゼニル(123I-IMZ)を用いて, Parkinson病(PD)15例を対象にPDの診断における123I-IMZによるSPECTの有効性を検討した. 123I-IMZ 167MBqを静脈内投与後約3時間にSPECT撮像を行い, 小脳皮質に対する各大脳皮質のSPECTカウントの比を結合能指標(BPI)として求めた. BPIとYahrのstage分類で判定した重症度を比較すると, 前頭葉上部を除く大脳皮質の各領域で有意な相関がみられた. 前頭葉上部においても有意ではないが同様の傾向がみられた. 以上の結果から, PDの疾患の進行に伴い, 大脳皮質のBZRの変化が進んでいることが示され, 123I-IMZ SPECTによりPDの病態をより詳細に把握できるものと考えられた.

Abstract[Summary]123I-iomazenil is a partial inverse agonist of centraltype benzodiazepine receptor(BZR). Single photon emission tomography(SPECT)using 123I-iomazenil was performed in 15 patients with Parkinson's disease(PD). SPECT images were obtained 3 hours after intravenous injection of 167MBq of 123I-iomazenil. The ratio of 123I-iomazenil SPECT count in the cerebral cortex to that in the cerebellum was calculated as the binding potential index(BPI). The BPIs in the lower frontal, temporal, parietal and occipital cortices correlated with the motor disability evaluated by Yahr's stage. The BPI in the upper frontal cortex showed the same tendency as in the other regions but did not reach a significant level. These results suggest that BZR in the cerebral cortex of patients with PD may be impaired in proportion to the severity of the disease and that 123I-iomazenil SPECT is useful for evaluating the pathophysiological condition in PD.
