Abstract | 「要旨」未治療原発性肺癌20例に対し保存的治療前後に201Tl SPECTを施行した. 治療後6か月以内の局所再発の有無により早期再発群と無再発群に分類し検討した結果, 治療後のRetention Index (Ret.I)は無再発群の方が有意に低値を示した(分散分析+Scheffe検定, p<0.005) が, 治療前のEarly Ratio(ER), Delayed Ratio(DR), Ret.I, 治療後のER, DR, 縮小率は両群間で有意差がなかった. PR群とNC群に分類した結果では両群間の治療前・後のER, DR, Ret.Iのいずれも有意差がなかった. 以上より原発性肺癌の保存的治療後の早期局所再発の予測は従来の腫瘍縮小率による治療効果判定では困難であるが, 201Tl SPECTの治療後のRet.Iが有用と考えられ追加治療の適応を考慮する上で有意義な指標になるものと考えられた. |

Abstract | [Summary]Thallium-201 single photon emission computed tomography (201Tl SPECT) was performed on 20 patients with primary lung cancer before and after treatment (radiotherapy alone or combination of chemotherapy and radiotherapy). They were divided into early recurrence group and no recurrence group by presence of local tumor regrowth within 6 months after treatment. Retention index (Ret.I) after treatment was significantly lower (p<0.005) in no recurrence group than that in early recurrence group. There were no significant differences in other items (early ratio(ER), delayed ratio(DR), and tumor size after treatment and all items before treatment). The results of this study showed that Ret.I after treatment was uniquely useful for prediction of early local recurrence of primary lung cancer. In conclusion, Ret.I after treatment in 201Tl SPECT indicated viability of residual mass, so it was useful in the evaluation of treatment effect for primary lung cancer. |
