Abstract | 「要旨」手術によって病理組織が確認された副甲状腺機能亢進症17症例において, 99mTc-MIBIによる副甲状腺病変の局在診断の有用性を検討した. 判定は, 2時間後の遅延画像から行った. 17症例21病変のうち18病変(86%)がMIBI画像で描出された. 検出最小例は, 腺腫で380mg, 過形成で270mgであった. 外頸・内頸動脈分岐部にあった異所性副甲状腺腺腫や副甲状腺癌術後の再発例では, 他の画像診断法では局在診断が困難であったが, MIBI画像で明瞭に描出された. これらの病変を含め, MIBI画像を他の画像診断法と比較すると, 検出率は201Tl-99mTcサブトラクション法で56%, 超音波検査で71%, CTで63%, MRIで67%とMIBIの検出率が勝っていた. MIBIシンチグラフィは副甲状腺病変の検出に優れており, 超音波検査等と組み合わせて行うべき核医学検査であり, 異所性および転移性病変のfirst choiceにすべき検査と考えられた. |

Abstract | The usefulness of 99mTc-MIBI scintigraphy for the detection of parathyroid lesions was evaluated in 17 patients with hyperparathyroidism. Delayed image was used to evaluate the lesions. Detectability of MIBI for parathyroid lesions was 86% (18/21). The smallest lesion detected was parathyroid hyperplasia weighted 270mg. Ectopic parathyroid adenoma and bone metastases of parathyroid carcinoma were clearly demonstrated. Detectability of MIBI scintigraphy for the lesions including ectopic and metastatic lesions was the highest among those of ultrasonography, CT and MRI methods. MIBI scintigraphy was thought to be useful for the detection of parathyroid lesions, especially for ectopic and metastatic lesions. |
