Abstract | 「要旨」甲状腺濾胞癌術後に骨, 肺, リンパ節等に多発性転移をきたした一症例に99mTc-MIBI(hexakis 2-methoxyisobutyl isonitrile)による全身腫瘍シンチグラフィを施行し, 細かい散布性肺転移巣を除き, 転移巣を明瞭に描出しえた. 99mTc-MIBI全身シンチグラフィは前処置不要で, 良好なシンチグラムが得られ, 甲状腺濾胞癌転移巣の検索に有用と思われた. 本症例は99mTc-MIBIにより分化型甲状腺癌転移巣を描出しえた最初の報告である. 「I. はじめに」分化型甲状腺癌術後の転移巣検出には131Iや123Iによる全身シンチグラフィのほか, 最近では201Tlによる全身シンチグラフィが臨床に広く用いられている. われわれは今回, 甲状腺濾胞癌術後に多発性の骨, 肺, リンパ節転移をきたした一症例に99mTc-MIBI(hexakis 2-methoxyisobutyl isonitrile)による腫瘍シンチグラフィを施行し, 転移巣把握に有用なる結果を得たので報告する. |

Abstract | [Summary] A 79-year-old male with a history of total thyroidectomy due to follicular thyroid carcinoma developed multiple metastases in the lung, bone, and lymph nodes. In order to detect and localize every metastases, 99mTc-MIBI whole body scintigraphy was performed, and clearly depicted all metastatic lesions except tiny pulmonary nodules. 99mTc-MIBI whole body scintigraphy may be useful in detecting metastases from differentiated thyroid carcinoma, because of a good quality of scintigram and no need of patient preparation. This article is the first report on 99mTc-MIBI localization in multiple metastases from differentiated thyroid carcinoma. |
