Abstract | 「要旨」過去10年間に施行した副甲状腺シンチグラフィ91例中59例で手術がなされ, 腺腫あるいは過形成と確認された57例について本検査法の診断率の分析を行った. 疾患の内訳は一次性38例, 二次性18例, 両者の合併が1例, また組織型では, 腺腫28例, 過形成28例, 両者の合併1例であった. 腺腫(30腺)の重量は0.100〜11.700g(1.840±0.446g(mean±SE)), 過形成(88腺)では0.025〜7.000g(0.623±0.100g)に分布していた. 全体, 一次性, 二次性, 腺腫, および過形成のsensitivityはそれぞれ47%, 69%, 34%, 87%, 33%, specificityは54%, 55%, 57%, 57%, 52%, accuracyは49%, 62%, 40%, 72%, 38%であった. 1g以下の描出率は, 全腺で39%, 腺腫および過形成ではそれぞれ94%および27%と腺腫で非常に良好であった. 本検査法は過形成よりむしろ腺腫の検出に有用である. |

Abstract | [Summary] 201Tl/99mTc subtraction scintigraphy (TTS) has been recognized as a useful procedure in the preoperative localization of hyperfunctioning parathyroid glands. Ninety-one patients suspected of hyperparathyroidism (HPT) were studied with TTS for the localization of lesions and 59 patients out of those had a neck exploration and parathyroidectomy which allowed for clinical correlation of pathologic findings with the imaging results. The weight of 30 glands of adenoma ranged 0.100 to 11.700g (1.840 +- 0.446g (mean+-SE)). The weight of 88 glands of hyperplasia ranged 0.025 to 7.000g (0.623+-0.100g). Overall sensitivity and specificity were 47% and 54%, respectively. The sensitivity for the adenoma and the hyperplasia was 87% and 33%, respectively. The specificity for them was 57% and 52%. There was significant difference for the detection between adenomas and hyperplasias. TTS is more useful for the localizing diagnosis of parathyroid adenomas than parathyroid hyperplasia in a patient with hyperfunctioning parathyroid state. |
