Abstract | 「要旨」虚血性心疾患32例に, 運動負荷/再静注201Tl心筋シンチグラムに加え, 再静注3時間後像を撮像し, 再静注3時間後像が心筋viabilityの評価に寄与するか否かを検討した. 方法は, 運動負荷201Tl心筋SPECTを施行し, 運動負荷初期像 (EX) を得, その3時間後に201Tlを再静注し, 再静注直後像 (Re1) を得た. さらに, 再静注3時間後に再静注遅延像 (Re2) を撮像した. 再構成したそれぞれのSPECT画像を13領域に分割し, 領域ごとに灌流低下の程度を視覚的に4段階にスコア化し, Re1とRe2で比較検討した. 結果は, (1)Exで灌流低下が認められた202領域中27領域 (13.4%) で, Re1に比しRe2でスコアが改善した. (2)Re1で心筋viabilityなしと判定した50領域中18領域 (36%) で, Re2でスコアの改善を認め, 心筋viabilityありと診断し得た. 以上より, 再静注法3時間後像は, 再静注像の心筋viabilityの過小評価を補う有用な方法であることが示唆された. |

Abstract | [Summary] Recent studies have demonstrated that some underestimation of myocardial viability remained to be not resolved even using reinjection method. So we devised a new technique of reinjection and reimaging, and using this we studied the detectability of myocardial viability in 32 patients of CAD. Exercise imaging (EX) was acquired after initial injection of thallium (111MBq) . Three hours after EX, the second dose thallium (37 MBq) was reinjected, and then the first reinjection image (Re1) was performed. Furthermore, the second reinjection image (Re2) was obtained at 3 hours after the second dose reinjection. SPECT images were divided in basal, mid and apical parts, and totally 13 segments were analyzed by visual scoring (0:severe, 1:moderate, 2:mild hypoperfusion and 3:normal) . Twenty-seven (13.4%) of 202 segments of low score (0 to 2) shown in EX were improved in Re2, compared with Re1. Twenty-one (42.0%) of 50 segments of score 0 or 1 shown in Re1 were improved in Re2. These results showed that our devised new method of reimaging 3 hours after the second dose injection was useful to improve in detectability of myocardial viability. |
