Abstract | 「要旨」原発性胆汁性肝硬変 (PBC) 15症例に対して99mTc-N-pyridoxyl-5-methyltryptophan (99mTc-PMT) 肝胆道シンチグラフィを施行し, 同疾患の肝機能および胆嚢機能の変化と組織学的進行度との関連について検討した. stage Iでは99mTc-PMT肝摂取率および肝排泄率の低下は認めなかった. stage IIからIVへと進行するとともに肝摂取率の低下を認め, 99mTc-PMT肝摂取率の算出はPBCにおける組織学的進行度の一指標となり得ると考えられた. 肝排泄率の変化についてはstageの進行との関連は乏しかった. PBCにおいては高頻度に胆嚢収縮率の低下を認め, またearly stageでも胆嚢収縮率の低下は認められ, 同疾患の病態を考えるうえで興味がもたれた. |

Abstract | [Summary] To define a relationship between the changes of hepatic and gallbladder functions and the pathological progression in patients with primary biliary cirrhosis, we studied 15 patients at various pathologic stages with 99mTc-N-pyridoxyl-5-methyltryptophan (PMT) hepatobiliary scintigraphy. In stage I both the hepatic uptake rates and hepatic excretion rates of 99mTc-PMT were not reduced compared with the normal subjects. According to the progress of the pathologic stage, the hepatic uptake rates of 99mTc-PMT were decreased. The hepatic excretion rates were reduced in stages II, III and IV, but they were not related to the progression of the pathologic stage. The hepatic uptake rate is considered to be useful in evaluating the pathological progression in patients with primary biliary cirrhosis. The mean gallbladder ejection fraction in patients with primary biliary cirrhosis (32+-18%) was significantly lower than that of the normal subjects (72+-13%) (p<0.005) . The reduction of the gallbladder ejection fraction was not related to the progress of the pathologic stage. Sluggish emptying of gallbladder may suggest the gallbladder wall lesion in this disease. |
