Abstract | 「要旨」 脳腫瘍の201TlCl取り込みは, 血流や血液脳関門の透過性のほかに, 能動輸送が関与する細胞摂取能に依存し, 細胞摂取能は細胞増殖率を反映するとされている. 201TlClを用いた脳SPECTで, 脳腫瘍診断上のパラメータの最適化について検討した. 対象は, 星状膠細胞腫(astrocytic tumor), 髄膜腫, 悪性リンパ腫, 下垂体腺腫, 転移性腫瘍等62例と対照の17例である. SPECT装置は, リング型SET-021(島津)を用いた. 静注5分後の早期に対する4時間後の遅延像の同一ROIによる腫瘍部SPECT値の比(Td/Tθ)が, 脳腫瘍の鑑別, 星状膠細胞腫の分化度の評価上最も有用であった. Td/Tθが, 従来報告の多い対側健常脳に対する腫瘍部の比(Tθ/Bθ)より優れる理由として, Td/Tθは, 容積効果, 吸収補正, および年齢に依存する健常脳の201TlCl動態等の誤差因子に左右されにくいからと考えられた. |

Abstract | [Summary] We performed 201Tl single photon emission CT (SPECT) studies on 62 patients with brain tumors prior to any therapy for the tumor. Ten lesions of ope-scar and 17 patients without brain tumor were also studied. The SPECT imaging were initiated 5 min (early image) and 4 hours (delayed image) after injection with 74 MBq of 201Tl-chloride. For a semi-quantitative analysis, we used the ratio of delayed count density to the early count density of the tumor (Td/Tθ), the ratio of tumor to normal brain tissue (Tθ/Bθ) on the early SPECT and the index of T/B ((Td/Tθ)/(Bd/Bθ)). Delay to early ratio of 34 normal brain tissue (Bd/Bθ) was 1.22+-0.15 and tended to decrease with the age. The Td/Tθ ratio of meningioma and region of operation scar was about 0.75 which was less than that of normal brain tissue. In 31 patients with astrocytic tumor, Td/Tθ ratio was clearly higher in poorly differentiated tumor than in well differentiated one. The index of Td/Tθ ratio was superior to Tθ/Bθ or T/B ratio for differentiating brain tumor and grading astrocytic tumor. |
