Abstract「要旨」肝細胞表面のasialoglycoproteinレセプタに特異的に結合する99mTc-GSAを用いて, 41例の慢性肝疾患患者(慢性肝炎11例, 肝硬変30例)を対象に肝シンチグラフィを行い, 種々の肝機能検査との関係を検討した. 99mTc-GSAの血中消失速度の指標となるHH15および肝集積率の指標となるLHL15は, 慢性肝炎と非代償性肝硬変の間で有意な差を示し, アルブミン量, プロトロンビン時間など各種肝機能検査値と有意の相関関係を示した. また, 一部の症例において99mTcO4-を用いた経直腸門脈シンチグラフィにより経直腸門脈シャント率を算出したところ, HH15およびLHL15との間に相関関係を認めた. HH15およびLHL15は新しい肝機能指標として臨床的に応用可能であると思われ, 99mTc-GSAは受容体を介した取り込みという新しい原理に基づく肝機能イメージング剤として, 臨床的に有用な検査法と考えられた.

Abstract[Summary]Liver scintigraphy using 99mTc-GSA, which binds specifically to the asialoglycoprotein receptors on the surface of the hepatocytes, was performed in 41 patients with chronic liver disease (11 chronic hepatitis cases and 30 liver cirrhosis) to investigate the correlations between the images and the various liver function tests. Two liver functional indexes, HH15 for plasma clearance of 99mTc-GSA and LHL15 for liver accumulation of 99mTc-GSA, showed significant differences between the chronic hepatitis group and the decompensated liver cirrhosis group. These indexes also showed significant correlations with the liver function tests such as serum albumin level and prothrombin time. HH15 and LHL15 correlated with the per-rectal portal shunt index obtained in 10 patients by 99mTcO4-per-rectal portal scintigraphy. These results suggest that HH15 and LHL15 have value as new liver functional indexes.
