Abstract「要旨」近年, 下肢静脈血栓症による肺梗塞の防止に, 下大静脈フィルターが使用されている. だが, 本例ではフィルターに塞栓症が生じ, これに対して強力な抗凝固療法を行った結果, 多発性の後腹膜血腫が生じた. 血腫の診断はCTで容易であった. RI venographyは大腿静脈血栓の描出に加え, 側副路も良好に描出しえ, 抗凝固剤の流入経路とこれに伴う血腫の分布の理解に有用であった. 下大静脈フィルター留置後の経過観察に, RI venographyは有用な検査法と思われた. 「I. はじめに」肺塞栓症は予後不良な急性疾患の一つであり, 下肢静脈塞栓症がその誘因であることは広く知られている. 下肢の静脈から肺へ血栓が移行するのを防ぐ有用な手段として, 近年下大静脈フィルターが利用されるようになってきた. しかし, 付着した血栓により下大静脈フィルターが閉塞し, 下肢静脈系の塞栓症が悪化することもまれではない.

Abstract[Summary]We presented a case of massive retroperitoneal hematoma owing to anticoagulant therapy for IVC filter thrombosis. CT showed the hematoma as high density area. RI venography demonstrated the thrombosis and collateral veins, suggesting the sites of anticoagulant induced bleeding and resultant hematoma. We suppose that RI venography is useful for the follow-up study of the IVC filter inserted cases.
