Abstract | 「要旨」記憶障害のみを有しその他の全般的知的機能障害を認めない健忘症候群の4例に123I-IMP SPECTを施行し, 局所脳血流分布の異常を定性的に検討した. 2例に側頭頭頂葉連合野を中心とした明らかな血流低下が観察され, その特徴的な血流分布のパターンからこれらはアルツハイマー型痴呆の病初期である可能性が推測された. 本法の痴呆患者における診断的有用性については, これまで多くの報告がなされているが, 本検討からアルツハイマー型痴呆の早期, 特に健忘症候のみを呈する病初期の診断にも有用であることが示唆された. 「I. はじめに」痴呆の症候には記憶障害をはじめとして抽象的思考や判断力の障害, 失語, 失行, 失認などの高次大脳皮質機能障害, 人格変化, さらに感情, 意欲, 行動の障害などがある. この中で, 記憶障害は痴呆の初期症状および主要徴候として診断のための必須条件でもあるが, これのみでは痴呆の診断とはなり得ない. |

Abstract | [Summary]Regional cerebral blood flow was studied using single photon emission CT (SPECT) with 123I-IMP to elucidate the pathophysiology of amnesia. Four patients with amnestic syndrome diagnosed by DSM-III-R criteria were investigated. SPECT images demonstrated definite hypoperfusion of the temporoparietal lobe and relative sparing of motorsensory and occipital cortices in two out of four patients. Since these abnormalities of regional cerebral blood flow have been considered to be characteristic findings in dementia of the Alzheimer type, two patients with temporoparietal hypoperfusion are suggested to manifest the early stage of dementia of the Alzheimer type. Our observations suggest that 123I-IMP SPECT is an useful modality in the diagnosis of the early stage of dementia of the Alzheimer type, especially in patients with only memory impairment. |
