Abstract「要旨」発症後1か月以内の虚血性脳血管障害でX線CTにより皮質下に長径2cm以下の小梗塞が検出された43例において, N-isopropyl-p-[I-123]iodoamphetamine(123I-IMP)SPECTを行い, 責任病巣側の低灌流域の広さと, 発症後6か月の機能的転帰(functional outcome)との関係について検討した. 機能的転帰はSPECTの低灌流域が広い症例ほど不良であった. (x2=29.3;p<0.001). SPECTの低灌流域の広さは頸動脈あるいは頭蓋内動脈の狭窄度と正の相関関係にあり(r=0.61;p<0.01), 機能的転帰が不良な症例では, SPECT, 脳血管造影のいずれにおいても異常を認めた. 皮質下小梗塞においてSPECTは, 穿通枝系動脈の閉塞によるlacunar infarctionと, 脳主幹動脈の閉塞あるいは高度狭窄による灌流圧の低下による梗塞の鑑別, 機能的転帰の推定に有用であることが結論できた.

Abstract[Summary]We performed 123I-IMP single photon emission computed tomography (SPECT) in 43 patients who had a small infarction (<2cm) in subcortical area and who were less than 1 month after onset of stroke. Hypoperfused area of brain was qualitatively assessed and was compared with functional outcome at 6-month after the stroke and the cerebral angiogram. Functional outcome was poorer in patients who had wider hypoperfused area in brain (x2=29.3;p<0.001). The extent of brain hypoperfused area showed a positive correlation with the degree of stenosis in the extracranial and/or intracranial arterics (r=0.61;p<0.01). In patients who had no angiographic abnormality, the extent and the location of the hypoperfused area were equivalent to that of the low density area in CT. Thus 123I-IMP SPECT in patients with a small infarction may discriminate lacunar infarction from embolic or hemodynamic infarction, which was caused by vascular lesions of major cerebral arteries, in subcortical area. Our study suggests that functional outcome is better in lacunar infarction than embolic or hemodynamic infarction in subcortical area.
