Abstract | 「要旨」随時尿中バゾプレシン (AVP) 濃度を測定し, その実測値と尿中クレアチニン補正値および尿浸透圧比を算出し, 尿崩症の鑑別に応用した. その結果, 中枢性尿崩症患児尿は尿中AVP濃度が著明な低値を示し, 健常児と異なった分布が認められた. また, 腎性尿崩症患児尿は, 単位浸透圧に対する尿中AVP濃度比に換算することによって, 健常児と異なった分布が認められた. 以上の結果より, 随時尿中AVP濃度の実測値と単位尿浸透圧比で比較することによって, 中枢性尿崩症, 腎性尿崩症と健常児の鑑別が可能であった. 「I. はじめに」脳下垂体後葉より分泌される抗利尿ホルモン (バゾプレシン : AVP) は, 9個のアミノ酸より構成されている分子量約1,000のペプチドである. AVPは, 血漿浸透圧の上昇や循環血液量の減少などの刺激によって分泌され, 腎の集合管および遠位尿細管の一部に作用して, 水の再吸収を行う重要な水・電解質の調節因子の一つである. |

Abstract | [Summary] The concentration of arginine vasopressin in unextracted random urine was directly measured by radioimmunoassay. The urinary AVP value (Vm) measured by RIA, their AVP value (Vc) revised by urinary creatinine and their AVP value (Vc / op) revised by urinary creatinine and urinary osmopressure were applied for discriminate diagnosis of diabetes insipidus (DI). In children with central DI, Vm was significantly lower then that of normal subjects and the distribution of Vm was different from that of normal subjects. In children with renal DI, the distribution of Vc / op was different from that of normal subjects. In conclusion the children with central DI and renal DI can be discriminated from the normal children in comparison with Vm and Vc / op. |
