| Abstract | 「要旨」左上大静脈遺残は大静脈奇形のうちではもっとも多く見られる. 多くは冠状静脈洞から右房へ還流するが, まれに左房へ直接開口する. 最近, われわれは心房中隔欠損を合併した左房に直接還流する左上大静脈遺残症例を経験した. 心RIアンギオグラフィはその血行動態の診断および心内短絡の存在を非侵襲的, 簡便に知る上でたいへん有用であった. 「I. はじめに」左上大静脈遺残は縦隔における静脈発生の先天奇形であり, 正常者の0.3〜0.5%に発生するといわれ, 大静脈の奇形のうちではもっとも頻度が高い. 多くは冠状静脈洞から右房へ還流するが, 稀に左房へ直接開口する. この場合多くは先天性心疾患を併発し複雑な血行動態を示す. 最近, われわれは心房中隔欠損を合併した左上大静脈遺残を経験し, その血行動態の診断に心RIアンギオグラフィが有用であった症例を経験したので若干の考察を加えて報告する. |

| Abstract | A persistent left superior vena cava (PLSVC) is the common clinical congenital abnormality of the greater central veins. This anomaly may occur as an isolated lesion, but rarely it is significantly associated with other congenital cardiac lesions, particulaly it drain into the left atrium. We recently encountered a patient in whom PLSVC to left atrial communication with atrial septal defect was diagnosed on the basis of radionuclide angiography with Tc-99m pertechnetate. Radionuclide angiography might be the noninvasive study of choice in patients suspected of having PLSVC by the clinical findings or chest roentgenography. |
