Abstract | 「要旨」 健常例3例, 虚血性心疾患6例の計9例に, 11C-パルミチン酸を用いたポジトロンCT (PET) を安静空腹時に40〜50分間行った. 血中からの排泄は速やかで, 全例5分以内に心筋が描出された. 左室心筋を6分割して得られた時間放射能曲線は二相性を示した. これより最大値 (Cmax) , 早期洗いだし時間 (Te 1/2) , 後期洗いだし時間 (T1 1/2) , 早期洗いだし分画 (%E) を算出した. Cmaxは梗塞心筋で低下したが (p<0.01) , Te 1/2, T1 1/2はばらつきが大きく, 健常, 虚血, 梗塞心筋で差はみられなかった (おのおの健常部18.8±6.0分, 81.4±36.0分 ; 虚血部19.3±5.8分, 66.9±25.7分 ; 梗塞部15.5士6.0分, 57.8±37.4分) . %Eはばらつきが大きいものの虚血心筋 (19.6±11.0%) では健常部 (26.6士9.8%) に比べて有意に低値を示し, β酸化の低下した心筋の存在することが示唆された. 以上より11C-パルミチン酸を用いたPETは安全で, かつ脂肪酸代謝の評価の可能性が示唆されたが, 安静時検査ではばらつきが大きく, 今後種々の負荷を含めた虚血の病態の検討が必要と考えられた. |

Abstract | [Summary] Serial dynamic images were obtained at rest following intravenous injection of 185-370MBq (5-10mCi) of 11C-palmitate for 40-50 minutes in 3 normal subjects and 6 patients with coronary artery disease. Due to the rapid clearance of activity from the blood, myocardial images were observed within 5 minutes after the injection in each case. The time activity curve of each myocardial region indicates biexponential clearance. The early clearance half time (Te 1/2) was similar in normal myocardium (18.8+-6.0min), ischemic myocardium (19.3+-5.8min) and infarcted myocardium (15.5+-6.0min) . The late clearance half time (T1 1/2) was also similar among these groups (81.4+-36.0, 66.9+-25.7 and 57.8+-37.4, respectively) . However, the residual fraction (%E) was slightly lower in the ischemic myocardium (19.6+-11.0%) than in the normal myocardium (26.6+-9.8%) (P<0.05) , indicating decreased beta oxydation in the former. Although positron emission tomography using 11C-palmitate may permit assessment of fatty acid metabolism, the parameters in this study were greatly fluctuated at rest among the normal, ischemic and infarcted myocardium. Further investigations under various interventions may be warranted. |
