Abstract | 「要旨」 放射線治療が施行された脳腫瘍17例と放射線治療後の脳障害の2例に201Tl, 123I-IMP-SPECTを施行しX線CT像と比較検討した. 17例中15例に治療前の造影X線CT像で濃染部位が認められ, 201Tl-SPECT像でもそれに一致する集積が認められた. 治療前後の比較では, X線CT像の濃染部位と201Tl集積には高い相関が認められた. しかし, 治療後に濃染部位が残存しても201Tl集積の認められない例があること, 放射線治療後の脳障害の1例も濃染部位は認められるが201Tl集積は認められないことから, 201Tlの集積は血液一脳関門の破壊のみでは説明できないと考えられ, 脳腫瘍の質的評価の可能性が期待された. 123I-IMP-SPECT像は201Tl集積部位の判定と腫瘍周囲脳組織の血流改善の評価に有用であると考えられた. 以上から, 201Tl, 123I-IMP-SPECT像は脳腫瘍の治療効果の判定と経過観察, および放射線治療後の脳障害と再発腫瘍との鑑別に有用であると考えられた. |

Abstract | [Summary] 201Tl, 123I-IMP-SPECT were performed in 17 patients with brain tumors before and after radiation therapy, compared with X-CT. 15 of 17 patients showed enhanced areas on X-CT and higher accumulations on 201Tl-SPECT in the same lesions before radiation therapy. There was significant correlation between enhanced lesions on X-CT and lesions of higher accumulation on 201Tl-SPECT before and after radiation therapy. But, the patients with residual enhanced lesions on X-CT did not always show higher accumulation on 201Tl-SPECT after radiation therapy. And, 2 patients with radiation injury after radiation therapy for brain tumor did not show higher accumulation on 201Tl-SPECT, though one patient showed enhanced lesion on X-CT. 201Tl-SPECT was expected to evaluate viability of brain tumor. 123I-IMP-SPECT was useful to recognize the site of higher 201Tl accumulation and evaluate blood flow around brain tumor. It is concluded that 201Tl, 123I-IMP-SPECT is useful for the evaluation of treatment effect of brain tumor and for the differentiation of cerebral radiation injury from recurrent tumor. |
