Abstract「要旨」 臨床的に急性散在性脳脊髄炎 (ADEM) と診断された症例の急性期と緩解再発期に, PETを施行し, 以下のような結果を得た. (1) 急性期には脳全体および局所の循環代謝の低下を認め, 二回目のPET測定ではこれらの改善と再発部位の循環代謝低下を認めた. これらの所見は臨床症状とその経過に一致した. (2) 本例のPET所見は多発性硬化症のPET所見との共通点を認めるものの, 脳全体の循環代謝の低下が認められる点が異なっていた. (3) 以上の脳循環代謝障害はCTでみられる形態状の変化より疾患の病態をよく反映し, 本症の病態および経過観察の上でPETが有用と考えられた. 「I. はじめに」 急性散在性脳脊髄炎 (Acute Disseminated Encephalomyelitis : ADEM) はウイルス感染症やワクチン接種後に炎症症状と, 意識障害を含む多彩な神経症状を呈し急性に経過する脱髄疾患であり, 多発性硬化症の類縁疾患と考えられている. 多発性硬化症のポジトロンCT (PET) による脳循環代謝測定は1, 2の報告がみられ, その病態把握や経過観察の上での有用性が検討されているが, ADEMに対するPET測定の報告はない. われわれは, ADEMと考えられる症例の急性期と緩解期にPETを追跡施行し, 興味ある知見を得た.

Abstract[Summary] Sequential positron emission tomography (PET) and single photon emission tomography (SPECT) were performed in an 11-year-old girl who was clinically diagnosed as acute disseminated encephalomyelitis (ADEM) . She had fever, stupor, loss of sight and left hemiparesis at administration on our hospital. X-ray CT (XCT) demonstrated a hypodense lesion in the right fronto-parietal white matter. In the 1st PET study, CBF and CMRO2 were reduced in both hemispheric white and gray matter, particularly in the right fronto-parietal lesion. The XCT abnormality was improved in the subsequent scan when the patient had improved except visual disturbance. When visual disturbance was worsened again, and a new hypodense lesion was appeared in the left frontal white matter on XCT, the 2nd PET study was performed. CBF and CMRO2 were recovered except the left frontal white and gray matter. SPECT study was also performed and the image was almost similar to CBF image of PET scan. The changes and distributions of CBF and CMRO2 were related to her symptoms and clinical course. Reduced CBF and CMRO2 of whole brain is thought to be characteristic of ADEM. From the experiences of this case, PET measurements is useful for the understanding of neuronal functional abnormalities of ADEM, and is more useful for the detection of recovery or relapsing Process than XCT.
