Abstract「要旨」 各種悪性腫瘍患者28例32病変に99mTc-HM-PAOによる腫瘍イメージングを試みた. 67Ga-citrate, 201TlCIとの比較も加え, HM-PAOの腫瘍血流量の評価の可能性を検討した. 32病変中19例 (59%) で明らかな腫瘍の描出が認められた. 組織血流量を表すRIアンジオグラフィの動脈相の放射能と, static imageの腫瘍部の集積の強度との間に強い相関が示された. また, 血流の豊富な腫瘍の周辺部に強く集積する傾向があった. これらの結果等から, HM-PAOによる腫瘍イメージングが主に腫瘍血流量を反映していることが示唆された. 従来の腫瘍イメージングとの比較では, 67Gaとは集積が一致しない症例が多かったが, 201Tlとは比較した4例全てに所見の一致を認めた. このHM-PAOの201Tlとの集積の類似性から, HMPAOの血管壁との親和性も集積に影響を与える可能性がある. HM-PAOは腫瘍血流量が評価でき, 悪性腫瘍の治療法の選択等への応用が期待される.

Abstract[Summary] Tumor imagings have been performed in 32 tumor sites in 28 patients with a variety of malignancies by using 99MTc-HM-PAO. They have been compared with radionuclide angiography and conventional tumor imagings such as 67Ga citrate or 201TlCl scanning, and then their possible values have been evaluated for the investigation of tumor blood flow. Tumors were clearly visualized in 19 out of 32 sites, with 59% of positive rate. Good correlation was observed between the radioactivity in the tumor site on static image of 99mTc-HM-PAO and the radioactivity at arterial phase on radionuclide angiography, which represents tumor blood flow. The radioactivities in the marginal regions tended to be stronger than those in the center of large tumors. These results indicated that tumor imaging by 99mTc-HM-PAO mainly reflected tumor blood flow. On the other hand, 99mTc-HM-PAO imagings were not well correlated with 67Ga-studies, while they were coincided with all 201Tl scannings performed. It suggested that the affinity of 99mTc-HM-PAO to capillary wall also involved in its accumulation in the tumor. In conclusion, tumor imaging by 99mTc-HM-PAO was able to estimate tumor blood flow, and this examination would be helpful for the management of malignancies.
