Abstract「要旨」 急性心筋梗塞における心筋のviabilityを明らかにするために, 18例の心筋シンチグラフィを検討した. タリウム-201の安静または運動負荷シンチ像と, その遅延像を追って記録した. 急性期の遅延像で, 9例に再分布所見 (50% : 再分布群) , 6例に悪化所見 (33% ; 逆再分布群) , 3例には変化がみられなかった (17% ; 無変化群) . その後の経過をみると, 逆再分布群では, 急性期から回復期, 慢性期にかけて, 遅延像の著しい改善がみられた. 再分布群と無変化群では, 有意な改善がみられなかった. われわれは, 急性期で逆再分布現象を起こす部位の心筋はviableであると結論した. 逆再分布を起こす部位では, 心筋の虚血性障害が, 冠動脈再開通により壊死から免れたのであろう. 「I. 緒言」 心筋梗塞・急性期に, タリウム心筋シンチ再分布現象を検討し, その後の経過を長期にわたって観察することで, 急性期における虚血心筋のviabilityを明らかにしようとした.

Abstract[Summary] To clarify the viability of myocardium in acute myocardial infarction, we examined 18 patients scintigraphically. They underwent rest or stress imaging and delayed imaging of thallium-201 during acute, convalescent and chronic periods. During acute period, a scintigraphic finding of the delayed filling in was observed in 9 cases (50% ; Redistribution group) . Worsening of the delayed image was observed in 6 cases (33% ; Reverse redistribution group) . No scintigraphic change of the perfusion defect was observed in 3 cases (17% ; No change group) . In reverse redistribution group, a remarkable improvement of the delayed image was observed through acute, convalescent and chronic periods. In redistribution group and no change group, no significant improvement was observed. We conclude that the myocardium of the reverse redistribution region during acute period may be viable. In the reverse redistribution region, recanalization of the coronary artery possibly protects myocardial damage from necrosis.
