Abstract | 「要旨」 In-111 oxineを用いて白血球を標識し, 骨系感染症またはその疑診例に投与して感染巣の診断とその活動性についての有用性を検討した. 感染positiveと判断された11例のうち9例に異常集積を認めたが, 1例では正常骨髄分布の欠損を, 1例は正常分布を示した. 感染が否定的であった10例のうち高度の関節炎の2例に集積がみられた. Ga-67シンチは感染群, 非感染群ともに5例中4例に異常集積が認められた. 骨シンチは16例について行ったがすべて陽性所見を示し, 急性化膿性脊椎炎, 慢性活動性骨髄炎または骨髄炎治癒後でも, 異常集積を示すため活動性の評価は困難であった. 骨系感染症では標識白血球によるシンチグラフィはsensitivityおよびspecificityともすぐれ, 臨床的に有用であると考えられた. |

Abstract | [Summary] In-111-oxine-labeled leukocyte imaging was performed on twenty-one patients suspected of having bone infection. Nine of eleven cases (82%) were diagnosed as having active infection as demonstrated by abnormal accumulation of In- 111-labeled leukocytes at the site of infection. There are two false negative (18%) cases. Two cases without active infection showed abnormal uptake. Four cases revealed cold defects on the scintigraphy. Marked uptake of radiotracer was noted not only in the case of acute osteomyelitis with acute septicaemia but also in the case with persistent chronic active osteomyelitis. It was observed that for precise evaluation of the test results it was equally important to compare the imaging findings with physical signs and laboratory investigations. It is concluded that In-111-oxine-labeled leukocyte imaging is a useful tool for the evaluation of the progression of bone infection. |
