Abstract「要旨」虚血性脳血管障害を対象に, 123I-IMP脳血流シンチの再分布現象redistribution phenomenonを, 局所脳血流量(rCBF)の観点から検討した. early SPECTは123I-IMP静注15分後に, delayed SPECTは5時間後に撮像し, 両者のimageからFilling-in rateとRedistribution rateを算定して分析した. その結果, (1)rCBFが低い領域ほどdelayed imageにおける相対的なcountは増加してくること(rCBFとFilling-in rateの逆相関). (2)CTで低吸収となる領域のrCBFは14.8±5.7ml/100g・脳/minであり, 再分布はほとんどみられないこと, (3)低吸収域周囲のrCBFは33.8±7.6ml/100g・脳/minで, 強い再分布がみられることなどが判明した. 総括的にいえば, rCBFが約20ml/100g・脳/min以上の領域では, それ以下の領域と比較すると再分布が強く現れる. 再分布は可逆的な虚血の程度と深い関係があると推測された. 以上の結果をもとに, 再分布現象を理解するためのシェーマを考案した.

AbstractRedistribution phenomenon on delayed SPECT image of N-isopropyl-p-[123I]iodoamphetamine (123I-IMP) was analyzed from the view point of clinical significance in ischemic cerebral vascular disorders. 123I-IMP SPECT was performed in 10 patients with chronic cerebral infarction whose lesions were identified in the cerebral hemisphere on X-ray CT. 123I-IMP was injected intravenously and regional cerebral blood flow (rCBF) was measured by the modified method of arterial blood sampling. Early SPECT was taken 15 min post IV injection, and delayed SPECT was performed 5 hours later. On the early image, ROI A was positioned in the area corresponding to low density area on CT and ROI B was in the opposit side (normal cortex). On the delayed image, ROI C and D were positioned at the same place as A and B, Filling-in rate and Redistribution rate (RD rate)were defined as follows, Filling-in rate=C-A/A×100% RD rate=(B/A)/B-(D-C)/D/(B-A)/B×100% As the result, inverted correlation was observed between rCBF and Filling-in rate. In the low density area (LDA) on CT, the rCBF was 14.8+-5.7 ml/100g・brain/min and RD rate was less than 50%. In the area around the LDA (low perfusion and normal density area), the rCBF was 33.8+-7.6ml/100g・brain/min and RD rate was more than 50%. Redistribution was most clearly observed in the area in which rCBF was in the range from 20 to 30ml/100g・brain/min, which is considered to be the threshold level of reversible ischemia. Therefore, redistribution was supposed to be closely related to the viability of cerebral parenchyma.
