Abstract「要旨」脊髄小脳変性症(SCD)患者11例に, 99mTc-HM-PAO SPECTを行い, 123I-IMP SPECT所見と比較, 検討した. また, 6例についてはTRHを負荷し, 集積の変化を検討した. SCDでは99mTc-HM-PAOの小脳/前頭葉の集積比は低下しており, しかも重症度に比例して, それは低下傾向を示した. 99mTc-HM-PAOと123I-IMP SPECTの小脳/前頭葉の集積比を比較すると, 11例中9例で123I-IMPの方が99mTc-HM-PAOよりも低値であった. TRH負荷により, 99mTc-HM-PAOの小脳/前頭葉の集積比の変化には, 一定傾向がみられなかった. さらに, 99mTc-HM-PAOと123I-IMPとでTRH負荷前後の小脳/前頭葉の集積比の反応のdiscrepancyがみられた. SCD患者では99mTc-HM-PAOと123I-IMPとの集積の違いやTRHに対する反応性の違いが認められ, 両者の集積機序を考える上で興味深い所見と思われた.

Abstract[Summary] 99mTc-HM-PAO SPECT was performed in 11 patients with spinocerebellar degeneration (SCD), and those findings were compared with 123I-IMP SPECT. In addition, changes of accumulation of 99mTc-HM-PAO after TRH loading were studied in 6 patients. In patients with SCD, cerebella/frontal lobes accumulation ratio decreased, and with increasing the severity of SCD its ratio decreased more remarkably. In comparative study of 99mTc-HM-PAO and 123I-IMP, it was shown that cerebella/frontal lobes accumulation ratio was lower in 123I-IMP than in 99mTc-HM-PAO in 9 of 11 patients. After TRH load, no definite change of cerebella/frontal lobes accumulation ratio was observed. Furthermore, discrepancy of accumulation ratio in 99mTc-HM-PAO and in 123I-IMP after TRH load was shown in 2 of 6 patients, Thus, it was shown that there existed the difference of accumulation and the discrepancy of response to TRH load of 99mTc-HM-PAO and 123I-IMP in patients with SCD. Therefore, these data will provide a useful information in evaluating the mechanisms of accumulation of 99mTc-HM-PAO and 123I-IMP.
