Abstract | 「要旨」脳血管障害患者20例を対象に99mTc-hexamethyl-propyleneamine oxime (99mTc-HM-PAO)を用いた脳血流SPECTを施行し, X線CT, 133Xeガス吸入法による局所脳血流像および123I-IMP SPECT像との比較検討を行った. 装置は, リング型ECT装置SET-020 (島津製作所)で, 99mTc-HM-PAOの370〜740 MBq (10〜20 mCi)を静注後30分と4時間にデータ収集を行った, 脳虚血の描出は16例でみられ, 123I-IMPと同様の良好なイメージとして認められた. 2例ではluxury perfusionと思われる高血流域の描出がみられた. 脳梗塞例の病巣部のRI分布は, 133Xe, 123I-IMPと良い相関がみられた. また, 4時間後のRI分布は, ほとんど変化がみられなかった. 全例に副作用はみられなかった. 99mTc-HM-PAOによる脳血流SPECTは, 脳血管障害例における脳血流状態の把握に有用な検査法と思われた. |

Abstract | [Summary] 99mTc-hexamethyl-propyleneamine oxime (99mTc-HM-PAO) cerebral perfusion SPECT (single photon emission computed tomography) was studied in 20 patients with cerebrovascular disease, and the images were compared with 133Xe local cerebral blood flow (LCBF) image and N-isopropyl-p-[1231] iodoamphetamine (123I-IMP) SPECT image. Data collection of SPECT was started at 20 minutes and 4 hours after injection of 370-740 MBq (10-20 mCi) of 99mTc-HM-PAO, using a circular detector array emission tomography system (SET 020, Simadzu Co.). Low activity areas on HM-PAO SPECT image were observed in 16 patients and the quality of the images were almost equal to those of 123I-IMP. Luxury perfusion was detected by 99mTc-HM-PAO in 2 patients, but not by 123I-IMP. The activity ratios of ischemic areas to opposite normal areas on 99mTc-HM-PAO image were correlated with those on 123I-IMP SPECT image (r=0.96), and 133Xe LCBF (r=0.89). The activity ratios did not change at 4 hours after the injection. It is suggested that 99mTc-HM-PAO SPECT was a useful method for the evaluation of cerebral perfusion in patients with cerebrovascular disease. |
