Abstract「要旨」左室拡張期指標に及ぼす心拍数の影響を検討するために, ペースメーカ植込み患者6例を対象に, 心拍数を50/分から120/分まで10ずつ増加させて心電図同期心プールシンチグラフィを施行した. 拡張期指標としては1/3FF・PFR・1/3FR-mean・%EFVについて検討したが, 心拍数増加に伴い1/3FFは著しく減少し, PFRは50/分から80/分心拍ではあまり変化しなかったが, 90/分心拍以上では著しく増加した. 1/3FR-mean・%EFVは著明な変化を示さなかったが, 心拍数増加に伴いおのおの減少傾向および増加傾向を示し, 特に%EFVについては症例間にバラツキが認められた. 以上の結果より安静時50/分から80/分の心拍数での拡張期指標としてはPFR・1/3FR-meanが適当と考えられた.

Abstract[Summary] In order to assess the influence of change in heart rate on left ventricular diastolic function indices, ECG gated cardiac pool study was performed in 6 patients with implanted programmable AAI pacemakers. Heart rate was changed by atrial pacing from 50 to 120 beats/min, every 10 beats/min. The filling fraction during first third of diastole (1/3FF), the peak filling rate (PFR), mean first third filling rate (1/3FR-mean) and early filling volume ratio (%EFV), being used as the indices of left ventricular diastolic performance, were assessed. In accordance with increase in heart rate, 1/3FF decreased significantly. PFR were fairly stable from 50 to 80 beats/min, but increased significantly from 90 to 120 beats/min. 1/3FR-mean and %EFV did not change significantly, but 1/3FR-mean showed decreasing tendency and %EFV showed increasing tendency as the heart rate was increased. %EFV was more changeable index than other indices among clinical cases. These results indicate that PFR and 1/3FR-mean were appropriate diastolic phase indices at rest.
