Abstract | 「要旨」左室駆出率35%以下の拡張型心筋症と虚血性心筋症のエルゴメータ運動負荷による左室反応についてRadionuclide ventriculography (RNV) を用い検討した. 拡張型心筋症26例では, 運動負荷により左室駆出率は安静時23±8%から運動負荷時24±8%と有意の変化を認めなかった. 一方対象群としての虚血性心筋症23例では, 左室駆出率は安静時26±4%から運動負荷時22±5%と有意に減少した. 左室駆出率が同程度に障害されているにもかかわらず左室局所壁運動障害の程度は, 拡張型心筋症の方が軽度であった. 運動負荷による左室局所壁運動の増悪は拡張型心筋症では9例 (35%) に, 虚血性心筋症では13例 (57%) に認められ運動負荷時両群の鑑別上注意を要した. 経過観察中死亡した8例の検討では, 運動負荷時左室駆出率が低下した8例中5例が死亡し, 低下しなかった18例中3例が死亡しただけであった. 運動負荷は拡張型心筋症の予後を推察する指標になるかもしれない. |

Abstract | [Summary] The supine bicycle ergometer exercise was performed in 26 cases with primary dilated cardiomyopathy (DCM: LVEF was 35% or less) and 23 cases with ischemic cardiomyopathy (ICM: LVEF was 35% or less) . Left ventricular ejection fraction (LVEF) and regional wall motion were analysed with radionuclide ventriculography. Although the same resting LVEF between DCM (23+-8%) and ICM (26+-4%) at rest, severity of regional wall motion was prominent in ICM. Exercise LVEF was 24+-8% in DCM decreased significantly to 22+-5% in ICM. Exercise induced regional wall motion abnormalities were detected in 9 cases (35%) with DCM and 13 cases (57%) with ICM. Severe regional wall motion abnormalities (akinesis or dyskinesis) were frequently seen in 40 segments of total 104 segments of left ventricle in DCM. During up to 6 years follow up period, 8 cases with DCM had died due to congestive heart failure. In 8 cases with DCM who showed decreased LVEF during exercise, 5 cases died. But only 3 cases in 18 cases without decreased LVEF during exercise had died. Exercise induced left ventricular dysfunction in DCM seems to be a poor prognostic sign. |
