Abstract | 「要旨」虚血性心疾患のTl-201心筋シンチのSPECT像の客観的判定方法を確立することを目的として前下行枝結紮犬を用いて, SPECT像上の虚血領域(欠損部, 虚血部)の内層と外層のTl-201濃度を測定し, 正常部のそれと対比して摂取比を算出した. 摘出心の同部位のTl-201濃度を求め, 摂取比を求めた. SPECT像と摘出心でのTl-201摂取比はr=0.97, p<0.001の良い相関を示した. SPECT像で欠損部の内層の組織像は全例壊死を示したのに対し, 外層の組織像は7例中3例に壊死, 3例虚血像, 1例が正常であった. 壊死部のTl-201摂取比は内層で0.43±0.15, 外層で0.53±0.12, 虚血部のそれは内層で0.76±0.12, 外層で0.71±0.06であった. 壊死部のTl-201摂取比は虚血部のそれと比べ有意に低値を示した. 以上より, SPECT像は心筋内Tl-201濃度を正しく反映し, その摂取比は壊死部と虚血部の判定に有用であり, その分岐点は大まかに0.6であった. |

Abstract | [Summary]The accuracy and the objectivity of interpreter of thallium-201 myocardial perfusion scintigrams for areas with definite diminished thallium-201 as compared to adjacent region, was assessed, because the visual thallium image evaluation was subjective and influenced by the experience of the interpreters. We performed thallium-201 myocardial scintigram in single photon emission CT with experimental infarct dogs at 7th day after the ligation of proximal left anterior descending coronary artery. Segmental concentration and segmental perfusion were quantitated by counting the emission and the samples of myocardial tissues from thallium-201 in normal area, ischemic border and ischemic center area. Each segment of the emission CT and the myocardial tissue were divided into inner and outer layer. The ratio of the counts of thallium-201 uptake of ischemic border or ischemic center compared to the normal area was calculated. Thallium-201 uptake ratio of the scintigram and the myocardial tissue correlated well (p<0.001, r=0.97). In myocardial scan defect, all of inner layers were determined histologically necrosis, in contrast, in 7 cases of outer layers, three cases were necrosis, another three cases were ischemia, the last case was normal. Thallium-201 uptake ratio of the segment showing necrosis was 0.43+-0.15 in inner layer and 0.53+-0.12 in outer layer. That of the segment showing ischemia was 0.76+-0.12 in inner layer and 0.71+-0.06 in outer layer. The above results suggest that the regional thallium-201 uptake ratio in the scintigram may be able to be detected whether it is histologically necrosis or ischemia. |
