Abstract | 「要旨」各種陰嚢部疾患が疑われた46例に, 陰嚢部シンチグラフィ(Radionuclide scrotal imaging, 以下RSI)を施行し, その有用性を検討した. 睾丸捻転では睾丸部に一致して円形のcold areaを認めたのに対し, 急性副睾丸炎ではhotに描画された. また, 睾丸腫瘍では腫瘍部への強いRI集積を認めたが. 陰嚢水腫では陰嚢内で睾丸以外の部分にcold areaを認めた. このように疾患ごとに特徴的なRSI像を得ることができた結果, よく似た臨床所見を呈する疾患の鑑別が容易となり, 治療方針の決定にRSIは非常に有用であった. また, 精索静脈瘤のRSI像を鈴木の分類に準じ静脈瘤の広がりに応じて3段階に区分し, 触診所見と比較した. 静脈瘤の広がりは, RSIにおいてより広範囲に捉えられたが, 静脈造影を加えた3者で比較し得た例ではRSIの方が触診よりも正確な広がりを示していた. 以上よりRSIは触診よりも静脈瘤の診断に鋭敏かつ正確な検査であると考えられた. |

Abstract | [Summary](1) Radionuclide scrotal imaging (RSI) was performed in 46 patients with various intra-scrotal lesions. The radionuclide images were well correlated with the clinical course or findings of them. Especially, it was recognized that RSI was very useful in differentiation of testicular torsion from acute epididymitis, and hydrocele testis from testicular tumors. (2) Furthermore, 16 cases of testicular varicocele were investigated by the method which could clearly classify them into 3 degrees corresponding to the grading of Suzuki's criteria on physical findings. The results also indicated that RSI could be a sensitive and useful method to prove testicular varicocele as well as ordinary venographic study even if in the case with subclinical manifestation. |
