Abstract | 「要旨」肝細胞癌52症例の局在診断が確定した径1cm以上の105腫瘤について, 67Ga腫瘍シンチの腫瘍検出率をSPECTとplanar imagingで比較検討した. 67Gaシンチでは, 肝シンチの腫瘤による欠損部に一致した67Gaの集積が, 周囲肝組織の集積と同程度以上の場合を, 集積positiveとして評価した. Planar imagingにおける検出率は64%であった. 腫瘤径別検出率は, 1-2cmでは25%, 2-3cmでは48%, 3-4cmでは50%, 4cm以上では98%であった. SPECT併用による検出率は75%であった. 腫瘤径別検出率は, 1-2cmでは32%, 2-3cmでは71%, 3cm以上では98%であった. 肝細胞癌の67Ga腫瘍シンチにおいては, planar単独に比較して, SPECTを併用することにより, 2-4cmの腫瘤の検出率が向上した. |

Abstract | [Summary]Fifty-two patients with hepatocellular carcinoma (HCC) including 105 masses of more than 1 cm were examined for detectability by conventional imaging in combination with single photon emission computed tomography (SPECT) using 67Ga-citrate. The degree of Ga-67 accumulation in the lesion of HCC was classified compared with the surrounding hepatic area. In conventional imaging, the detectability was 64% in all, 25% in 1-2 cm, 48% in 2-3 cm, 50% in 3-4 cm, 98% in more than 4 cm in tumor size. In combination with SPECT, the detectability was 75% in all, 32% in 1-2 cm, 71% in 2-3 cm, 100% in 3-4 cm, 98% in more than 4 cm in tumor size. In HCC mass of 2-4 cm in size, efficacy of adding the SPECT was definitely helpful in detectability. Adding the SPECT was able to increase the difference of Ga-67 concentration in HCC and surrounding liver tissue, and also improve new detection, compared with conventional imaging only. |
