Abstract | 「I. はじめに」N-Isopropyl-p-[123I]Iodoamphetamine(以下123I-IMPと略す)は初回循環で高率に脳組織に取り込まれ, その分布が局所脳血流を示す薬剤として使用されている. 脳腫瘍の123I-IMP imageは低集積像として得られることが多く, 必ずしも腫瘍血流を反映していないことが指摘されており, その原因として腫瘍におけるreceptorまたはretention mechanismの欠如が推測されている. 一部には高集積像となる例も報告されており, 123I-IMPのextraction efficiencyの高いことがその原因として推測されている. いずれにしても, 脳腫瘍における123I-IMPのextractionとwashoutに関しては不明の点が多い. この研究の目的は腫瘍部における123I-IMPのextractionとwashoutを検討し, 経時的123I-IMP imageと腫瘍血流の関係を明らかにすることにある. |

Abstract | [Summary]81m-Kr SPECT and sequential 123I-IMP SPECT were performed in 5 cases of anaplastic glioma and 1 case of meningioma. An intra-carotid injection method permitted to evaluate 123I-IMP extraction and washout separately. In early images, 123I-IMP was extracted not only in normal area but also in perifocal lucent and contrast enhanced areas on X ray CT in the same ratio as 81mKr. Both contrast-enhanced to normal (ce/nl) and perifocal to normal (pfl/nl) ratios showed good correlations between early 123I-IMP images and 81mKr images. In anaplastic glioma, the contrast enhanced area was noted to be hot in 2 cases and to be cold in the University School of Medicine remaining 2 cases in early images. The washout rate of 123I-IMP was higher in the following order: contrast enhanced area, perifocal lucent area and normal area. It was considered that the hot area in early images changed to be cold subsequently. Contrary to anaplastic glioma, no significant difference of the washout rate of 123I-IMP was observed between the tumor and normal tissue in meningioma. It is supposed that the washout rate of 123I-IMP is in close relation to the type of brain tumors. |
