Abstract | 「要旨」甲状腺機能亢進症治療前後における, 肺機能と換気血流状態の変化について検討した. 肺機能検査では, 治療後に, 肺活量(VC)の増加と残気量(RV)の減少を認めた. 換気シンチグラフィによる換気分布は治療前後で, 有意な変化は認めなかった. 肺血流シンチグラフィでは, 甲状腺機能亢進症の91%に上肺野の血流増加を認め, それに伴って上肺野における換気/血流比は正常に比べ低下していた. 治療後, 甲状腺機能の正常化とともに上肺野の血流増加も正常化した. 肺活量の増加と残気量の減少は, 握力の改善より推測するに, 治療後に胸郭の収縮力, つまり呼吸筋の筋力が回復したことによると考えられ, また肺血流分布の改善は, 心拍出量の正常化によると考えられた. 「I. はじめに」甲状腺機能亢進症の肺機能に関する研究は1920年代より, 呼吸器機能検査の進歩とともに行われてきたが, 肺換気血流動態よりその病態を検討した研究は少ない. |

Abstract | [Summary]Pulmonary functions of the hyperthyroidisms were evaluated before and after therapy. Kr-81m continuous inhalation and Tc-99m MAA (Macro Agguregated Albumin) perfusion study, spirography (VC, RV, FRC, TLC, CV), Diffusion capacity, and flow volume (V25, V50) study were done before and after therapy of 32 hyperthyroidisms. As a result VC % increased and RV % decreased significantly after the therapy, due to improvement of weakness of respiratory muscle. No significant changes were recognized on FRC, TLC, CV, DLco, V25, V50 after the therapy. Perfusion shifted to the upper lung fields in 21 (91 %) out of 23 hyperthyroidism patients. There was no significant change of ventilatory distribution after the therapy. V/Q ratio in upper lung fields therefore decreased in hyperthyroid state. After the therapy, perfusion in upper lung fields recovered to normal pattern, corresponding to normalization of T4 value. Shift of perfusion to upper lung fields was considered to be one of pathophysiological changes of hemodynamics in the hyperthyroidisms. |
