Abstract | 「I. はじめに」右室負荷疾患における201Tlの右室壁への集積が右室負荷の程度をよく表していることは以前のわれわれの報告を含め多くの論文がある. その後の報告でplanar像での定量性についての検討も加えられている. Single photon emission CT(SPECT)の定量性に関しては未だに未解決の問題も多く残されているが, planar像と比較して放射能分布の定量的評価に関しては原理的には優れているといえる. われわれは右室圧負荷疾患における右室壁への201Tlの集積をSPECTを用いて定量的に解析し, 同時期に施行した心臓カテーテル検査での右室圧負荷の程度との相関を検討し, 良好な結果を得たので報告する. 「II. 対象」対象は当院において心筋SPECTと心臓カテーテル検査を行った右室圧負荷疾患10例および対照群としての正常右室圧7例であり, 右室圧負荷疾患の内訳は原発性肺高血圧症5例, 僧帽弁狭窄症3例, 肺塞栓症1例および肺高血圧を合併した進行性全身性硬化症の1例である. |

Abstract | [Summary]Correlation between Tl-201 right ventricular accumulation in SPECT and right ventricular systolic pressure (RVSP) was evaluated in 10 patients with right ventricular pressure-overloading and in 7 normal pressure patients with other nonspecific diseases. Myocardial SPECT studies were performed in the resting state 10 minutes after intravenous administration of 3 mCi of Tl-201. A digital gamma camera with a parallel hole collimator and energy window of a 80 keV +- 30 % was used. 60 projections were obtained in every 3 degree's step (180 degree rotation): the data sampling was performed in 64 matrix size with acquisition time of 15 seconds. The reconstruction filter was the Chesler's one and no attenuation correction was adopted. On a short axis slice of 16 to 32 mm thickness, polygonal ROIs were chosen around both free wall so as to include as much myocardial volume as possible. The integration of counts in the ROI was divided by the wall length in the ROI: the value means average thallium uptake per wall length. The SPECT RV/LV uptake ratio was defined as the ratio of both ventricular free wall accumulation per wall length, and we have found out it has a excellent correlation (r=0.96) with right ventricular systolic pressure. In particular in the cases of comparatively mild right pressure-overload. The SPECT RV/LV uptake ratio exhibited fairly good sensitivity and was able to separate them from normal pressure cases. As a conclusion, Tl-201 myocardial SPECT was shown to be a useful and sensitive method for assessing right ventricular pressure overloading. |
