Abstract | 「要旨」心電図同期Single Photon Emission Computed Tomography(SPECT)による左右心室容積の算出法の検討を行った. 心室輪郭は等カウント法にて決定し, その際のカットオフ値(C)はファントム実験にて得られた以下の式に従って決定した. C=0.47・BG+44.2(%)(BG:心室内最高カウントに対するバックグラウンドの%値). 心電図同期SPECTによる左室容積(Y)と左室造影による(Area-Length法)左室容積(X)とを比較したところ, Y=0.96X+6.6(ml), r=0.983, n=51と良好な一致を示した. 短絡および逆流疾患のない20例においての左右心室一回拍出量比(SVR)は1.03±0.08(SD)とほぼ理論値を示した. 左室容積が正確に測定されたことと, SVRが理論値を示したことより, 右室容積も正確に測定されたと考えられた. 以上より, SPECTによる心室容積算出法は採血の不要な信頼できる方法と結論された. |

Abstract | [Summary]A new method for the measurement of right and left ventricular volume using single-photon emission computed tomography (SPECT) was evaluated in patients with various cardiac diseases. Absolute ventricular volume was calculated based on the number of boxels above a threshold of a maximum count within a volume of interest. The optimal threshold level (OT) for delineating ventricular border was determined by the following equation: OT= 0.47・BG + 44.2 (%), where BG is background level expressed as the ratio (%) of background to maximum ventricular count in the reconstructed image. In 26 patients the left ventricular end-diastolic and end-systolic volumes measured by SPECT (Y) showed excellent correlation (r=0.983, p<0.001) with those by single-plane left ventriculography (X) (area-length method): Y= 0.96X + 6.6 (ml), n=51. Stroke volume (SV) ratios calculated in 20 patients without shunt and valvular diseases were 1.03 +- 0.08 (SD). Since left ventricular volumes were measured accurately and SV ratios showed theoretical values, right ventricular volumes were also considered to be measured accurately. It is concluded that our new method for the measurement of right and left ventricular volumes using SPECT was accurate and reliable technique. |
