Abstract | 「要旨」褐色細胞腫が疑われた28症例に30回のm-[131I]iodobenzylguanidine(I-131MIBG)シンチグラフィーを行った. そのうち確定診断の得られた24検査例についてI-131MIBGシンチグラフィーの意義と診断能についてコンピュータ断層撮影(CT), 超音波断層撮影(US)と比較し検討した. I-131MIBGのsensitivityは8/9(88.9%), specificityは15/15(100%), accuracyは23/24(95.8%)であった. 一方, CTのsensitivityは9/9(100%), specificityは10/15(66.7%), accuracyは19/24(79.2%)であり, USのsensitivityは8/9(88.9%), specificityは12/15(80.0%), accuracyは20/24(83.3%)であった. 褐色細胞腫の診断に際しI-131MIBGシンチグラフィーはCTに比べ腫瘍の大きさの面で限界があり, 今回の症例では検出限界は2.0×2.5×2.0cmであったが, CTやUSに比し, より特異性の高い検査法であることが示された. |

Abstract | [Summary]Scintigraphic images using I-131 MIBG were reviewed in the 28 patients with suspected pheochromocytoma. The diagnostic efficacy of the I-131 MIBG scintigraphy for location of mass was discussed comparing with computed tomography (CT) and ultrasonography (US). The smallest size of the pheochromocytoma revealed by both I-131 MIBG scintigraphy and US was 2.0 × 2.5 × 2.0 cm. On the other hand, CT detected smaller mass, 1.0 × 1.5 × 0.4 cm in size. The sensitivity was 88.9% in I-131 MIBG scintigraphy, 100% in CT, and 88.9% in US. The specificity was 100% in I-131 MIBG, 66.7% in CT, and 80.0% in US. And the accuracy was 95.8% in I-131 MIBG, 79.2% in CT, and 83.3% in US. Thus I-131 MIBG scintigraphy was proved to be more sensitive than both CT and US for the location of pheochromocytoma, and is an available method for the first choice. |
