Abstract | 「I. 緒言」Angiotensin IIはNoradrenalineの10倍の血管収縮作用を有し, かつ冠血流量にそれほど影響を与えることなくafterloadを上げる昇圧物質で, その半減期は15〜20秒と短く, 臨床上安全に使用できる. 半減期が短いためafterload負荷, 解除のcontrolも迅速に行うことが可能である. 一方, 核聴診器(nuclear single probe)は, プローブ型検出器を用い, リアルタイムで10msecという高時間分解能を有する左室容積曲線が得られ, 12秒で情報収集が終了する. さらに短時間(約100心拍)のmultibeat法を応用すれば, 拡張期の指標が得られ, 信頼性も向上する. 今回の研究のように, 短時間で左心負荷を加えたり解除して心機能の反応特性を検討する場合, 以前よりの心プールイメージによる解析に優る長所を有している. さらにコリメータを一度設定すれば一回の注射で長時間の観察が可能である. |

Abstract | [Summary]Effects of angiotensin II on cardiac function were assessed in patients with heart disease by nuclear single probe. The study population included 6 normal volunteers (Group A), 3 patients with hypertension (Group B) and 11 with left heart disease (Group C). Left ventricular functions were determined non-invasively using technetium-99m and single probe technique before, during and after intravenous administration of angiotensin II. Heart rate during angiotensin II decreased in Groups A and B while it increased in Group C. Left ventricular ejection fraction, cardiac output and peak ejection rate were lower in Group C than in Groups A and B before angiotensin II infusion and decreased during angiotensin II in all three groups. In Group C these values increased after angiotenin II infusion compared to the values before angiotensin II with no significant change of peak filling rate. Group C was differentiated from others by systolic values during and after angiotensin II. The differences of effects of angiotensin II between Group C and Groups A and B suggest that this method may be useful to assess cardiac functions in various heart diseases. |
