Abstract「要旨」食道動態シンチグラフィーの有用性につき検討した. 対象は, 健常成人(正常群)34例, 食道炎症状を呈し上部消化管内視鏡を施行した例(食道炎群)16例である. 方法はTc-99m DTPA 0.3mCiを嚥下させ, 以後15秒ごとに40回嚥下運動を繰り返させた. 食道に関心領域をとりこの時間放射能曲線を作成し, t回目のクリアランス率CtをCt=(Emax-Et)/Emax×100(Et:t回目の15秒間の放射能, Emax:Etの最大値)としクリアランスカーブを作成した. 正常群, 食道炎群のそれぞれのクリアランスカーブの平均を求めた. 食道炎群は正常群に比べて食道運動機能の低下, すなわち食道のクリアランスの低下があると考えられた. 異常例に対して病態に応じて与薬を行いクリアランスカーブの正常化があった. 本検査法は簡便かつ安全であり, より生理的に軽度の食道運動機能異常を検出しうると考えられた. また, 病状の変化の追跡にも有用と考えられた.

Abstract[Summary]We have evaluated the usefulness of the esophageal scintigraphy. Study subjects included 34 normal volunteers and 16 patients with symptoms of esophagitis, who had been evaluated endoscopically beforehand. Three hundred microcurie of 99mTc-DTPA solution was administered orally, which was followed by serial swallows at 15 sec. intervals for 40 times. Count rates over the esophageal area of interest were obtained and the clearance curve was made with the following formula: Ct = (Emax -Et)/Emax × 100 where Ct represents percent esophageal transit at time t; Emax, the maximal count rate; and Et, the count rate at t. The following 2 indices were obtained from clearance curves. The 1st index was total clearance; average Ct from C36 to C40. The 2nd one is early clearance index; a minimum number of swallows requiring 80 % of total clearance. The 2 indices were compared between the normal subjects and patients with the symptoms of esophagitis. The total percent clearance of the patients with the symptoms of esophagitis was significantly lower than that of normal subjects. The early clearance index of the patients with symptoms of esophagitis is delayed in comparison with that of normal subjects. These indices were more sensitive in diagnosis of esophagitis than endoscopy. Among patients with deranged clearance curves, normalization of each curve was observed after the appropriate therapy.
