Abstract「要旨」: 運動負荷心筋イメージにおけるThallium-201 (201Tl) 肺野集積度が非観血的な負荷時心機能評価の指標となり得るかを調べるため, 冠動脈造影を施行した58例を対象に運動負荷および安静時心筋イメージングを行い, うち19例に同一負荷量での心血行動態検査を施行して, その結果と対比した. 負荷時201Tl肺野集積度の指標としては左上肺野対心筋部位カウント比の安静時より負荷時への変化率, 201Tl肺野集積度変化率 (%ΔLUI) を用いた. %ΔLUIは最大負荷時の肺動脈楔入圧, 心拍出係数, 1回拍出係数との間にr=0.82, r= -0.66, r= -0.47 (p<0.001, p<0.01, p<0.05) の相関を認めた. また%ΔLUI高値を示した症例は多枝病変, 梗塞合併を高率に認め, %ΔLUIと梗塞サイズ, 左室駆出率との間には有意の相関を認めた (r=0.50 p<0.05, r= -0.65 p<0.001) . 以上より%ΔLUIは負荷時の心ポンプ機能不全と冠動脈病変重症度をよく反映し, 虚血性心疾患の負荷時心機能を推定する非観血的指標として有用であることが示された.

Abstract[Summary] : To clarify whether an increase in lung Thallium-201 (201Tl) uptake after exercise may reflect impaired cardiac function, 201Tl myocardial imaging was performed after exercise and at rest in 58 patients (pts) who underwent coronary angiography. Hemodynamic measurements were carried out at rest and peak exercise in 19 of 58 pts. The ratio of left upper lung to maximal myocardial activity (LUI) was calculated as an index of 201Tl lung uptake, and percent change of LUI from rest to exercise (%ΔLUI) was adopted as an index of lung 201Tl activity after exercise. %ΔLUI significantly correlated with pulmonary capillary wedge pressure, cardiac index and stroke volume index at peak exercise (r=0.82, r=-0.66 and r=-0.47 respectively) . Pts who showed higher %ΔLUI than -17% (1 SD above the mean of pts without significant coronary artery disease) had higher prevalence of old myocardial infarction and multivessel disease compared with pts who showed lower %ΔLUI. And %ΔLUI also significantly correlated with infarct size determined by myocardial image at rest (r=0.50) . These results indicate that %ΔLUI is useful as a noninvasive index for assessing cardiac reserve at exercise.
