Abstract | 「I. はじめに」Factor Analysisはダイナミックデータの数学的処理により対象臓器の機能成分を抽出し, この機能成分を用いて対象臓器の形態成分を求める新しいRIの動態解析法である. 最近, 心臓, 肺, 腎臓など各種臓器に臨床応用されるようになってきたが, われわれは99mTc-DTPAによる移植腎の動態解析に応用し, Factor Analysisの有用性を検討したので報告する.「II. 対象および方法」正常2症例と自家腎移植6例を含む腎移植13症例を対象とした. データ収集は99mTc-DTPA 370〜555MBq (10〜15mCi) を静注後, 正常例では後面から, 腎移植例では前面からγ・カメラZLC-7500 (Siemens社製) にて撮影し, 初期の血流相では2秒ごとに30フレーム, 機能・排泄相では30秒ごとに60フレームをScintipac-2400 (島津製作所) に64×64マトリックス・サイズ (視野 : 38.7cmφ) で入力した. これらのデータをもとに, まず初期の血流相のデータを30秒ごと2フレームに加算し, 機能・排泄相のデータと同様にし, フィルター処理 (Butterworth filter) を行い, さらに腎臓部分を約2.7倍に拡大し, Factor Analysisを行った. なお, Factor AnalysisのソフトウエアはBarberおよびDi PaolaらのAlgorithmを利用したものである. |

Abstract | [Summary] : The clinical usefulness of factor analysis was assessed in renal dynamic studies using 99mTc-DTPA. The optimal conditions for factor analysis of renal dynamic studies were decided in normal kidneys, and then transplanted kidneys were examined with these conditions. The results were summarized as follows : 1) When normal renal images were magnified 2.7 times and compressed into 8×8 dixels, it was suitable for factor analysis to extract three factors from 24 or 32 dixels. 2) Since the factors of vascular, renal and renal pelvic phases were automatically extracted by factor analysis, it was possible to obtain more detailed informations from these factor images than conventional renograms. 3) Factor analysis was useful for follow-up studies in patients with transplanted kidney and the contribution ratio of each factor was considered to be one of the indices evaluating the sick conditions. |
