Abstract「要旨」: I-131-metaiodobenzylguanidineは, 褐色細胞腫の陽性描出剤として欧米の多くの施設においてその有効性が証明されている. 今回われわれは, 褐色細胞腫を疑われた患者24名にI-131-metaiodobenzylguanidine (MIBG) シンチグラフィを施行し, 右副腎原発2例, 傍大動脈部交感神経節原発1例, 膀胱部原発3例の腫瘍描出を認めた. このほか, false positive, false negativeを各1例認めたが, 全体で, sensitivityは85.7%, specificityは94.1%であり, I-131-MIBGが褐色細胞腫の診断に有用であることが示された. また, 文献的には20%以下といわれている正常副腎の描出を24例中15例 (62.5%) に認めた. われわれの施設で高率の副腎描出を認めた明確な理由は不明だが, 評価基準, 撮影条件, 対象の体格の違いが関与していると推定され, 副腎描出の病的意義には検討を要すると思われた.

Abstract[Summary] : A number of investigaters, both in the United States and Europe, have reported I-131-MIBG scintigraphy to be efficacious means of locating pheochromocytoma. In this study, 24 patients suspected pheochromocytoma were examined by I-131-MIBG scintigraphy. Tumor uptake was shown in 6 cases, two at the adrenal gland, three at the vicinit of the urinary bladder, and one at the paraaorta. Those underwent operation, and all of the tumors were histologically proven to be pheochromocytoma. We also obtained one false negative and false positive case. An overall sensitivity was 85.7%, and the overall specificity was 94.1%.This result suggests that I-131-MIBG scintigraphy is sensitive indicator of pheochromocytoma as considerable studies have already demonstrated. On the other hand, we found the ratio of visualization of normal adrenal medulla in our institute was much higher than that in other series. We are unable to explain the reason, but several factors may be considered. However, further discussion on the pathological meaning of the adrenal uptake will be needed.
