Abstract「要旨」: 心拍同期心プールシンチグラフィから左心室輪郭を完全自動認識することにより, 心駆出率, 定量的壁運動および, 左室容積を求める一連の左心機能解析法を開発した. 本論文では, この左心機能解析の中心となる左心室輪郭自動認識の方法論について報告する. 本法は, 以下のような特徴を有する. (1) 左心室中心より放射状プロフィールを作成し, フーリエ級数近似して各プロフィールごとに変曲点または閾値点の輪郭候補点を求めた. 輪郭候補点から輪郭点を決定するのに評価関数を導入した. 各輪郭点は, 補正後に極座標系でフーリエ級数近似することで滑らかに接続して左心室輪郭を作製した. (2) 複数個の患者データに対して連続処理をし, 心駆出率を求め, 処理結果をすべて保存し, 輪郭抽出結果より, 定量的壁運動, 左室容積を算出可能とした. (3) 237例の臨床データについて検討し, 自動認識の成功率は, 87.2%であり, コントラスト左室造影法による心駆出率との相関性により処理精度を確認した.

Abstract[Summary] : A left ventricular (LV) function analyzing method has been developed to detect the LV contour from gated blood pool imaging. Ejection fraction (EF) , LV volume and quantitative wall motion are calculated. The basic algorithms for fully automatic LV contour detection are described. The characteristics of the method are summarized as follows : 1. A rectangular box involved LV is determined by the horizontal and vertical profile. The LV center is located in the box. 2. From the LV center, a radial profile is generated and fitted to Fourier function. Some edge points (the point of influction or the threshold point) are derived on each radial profile. 3. By using an evaluation function, the optimum contour point is determined. 4. The LV contour is formed by smoothly connecting all the contour points by fitting them to Fourier function. 5. A sequential process (EF calculation) of many patient data is available and all the results are saved. 6. By using the saved LV contour, quantitative wall motion and LV volume are calculated. 7. Success score of the method is 87.2% as assessed from 237 studies. 8. Comparative data from the scintigraphic and the contrast left ventricular angiography are presented.
