Abstract「要旨」N-isopropyl-p-123I-iodoamphetamine(123I-IMP)による脳血流のダイナミックスキャンを行った. 対象症例は, 原発性脳腫瘍13例, 転移性脳腫瘍3例, 脳動静脈奇形(AVM)1例, 正常3例の合計20例である. 装置は島津製リング型ECT装置(SET-020)で, データ処理は日本データジェネラル社製ミニコンピュータシステム(ECLIPSE S-120)にて行った. また, ほぼ同時期に133Xe吸入法による局所脳血流(rCBF)の測定も行った. 検討方法は, 123I-IMP静注後0〜20分における病変部と対側皮質領域のactivityとの比(T/N比)を求め, 腫瘍別にT/N比のカーブを比較し, また, 133Xe吸入法によるrCBF像でのT/N比と比較した. 正常例では, 静注後の立ち上がりは急速で30分後にはpeak値の90%に達した. 脳腫瘍例では, 星細胞腫, 下垂体腫瘍, 松果体腫瘍, 転移性脳腫瘍において0〜20分の間はほぼ一定のT/N比を示し, その値は40〜100%と大きく異なるものの腫瘍の種類により一定の傾向はみられなかった. 髄膜腫, AVMでは早期においてT/N比が高く, 123I-IMP静注後2分におけるT/N比は, 133XeによるT/N比と高度の相関がみられ, 123I-IMP静注後早期の像は病変部の血流程度を現わしていた.

AbstractDynamic sequential scintigraphy using I-123-IMP were performed in 3 normal individuals and 17 patients with cerebral mass lesions. Instruments used were ring type ECT (SET-020, Shimadzu Co. ) with minicomputer system (ECLIPSE S-120, Japan Datageneral Co. ). Sequential scintigraphy was carried out immediately after injection of 1 mCi of I-123-IMP with the frame mode. The activity ratio of the tumor to cerebral cortical region of the opposite side (T/N ratio) was calculated in each frame. The activity of cerebral cortex and basal ganglia increased rapidly, and activity in the white matter is about 80% of the gray matter in 60 min. T/N ratios in the patients with astrocytoma, metastatic brain tumor, pituitary tumor and pineal tumor were constant in 0-20 min after injection (40-100%) and were not related with tumor type. T/N ratios in meningioma and AVM were significantly high in early stage, and showed good correlation with those of rCBF image obtained by Xe-133 gas inhalation method (correlation coefficient is 0.97), which indicates rate of blood flow in the mass lesion. Dynamic scintigraphy may be a useful method to evaluate the uptake mechanism of IMP in the tumor.
