Abstract「要旨」運動負荷Tl-201心筋シンチグラフィの短軸断層像を用いて各スライスのcircumferential profile解析を行い, 心筋血流分布, 洗い出し率を機能マップとして表示した. また, 標準マップを用いて, 最大運動時, 再分布時および洗い出し率の異常の判定を行い, 異常の広がりおよび程度を判定するextent mapおよびseverity mapを作製した. さらに定量化のため, extent scorc (Ex-S) およびseverity score (Sv-S) を算出した. 本法を冠動脈疾患51症例に応用したところ, 各マップは狭窄動脈と良い相関を示し, 異常の領域の3次元的理解も容易になった. Ex-S>0.10, Sv-S>5.0を異常の判定基準にすると有病正診率45/46 (98%), 無病正診率5/5 (100%) と良好な診断率を得た. また洗い出し率の絶対値は運動負荷量および狭窄動脈数と有意の相関を示した. 本法は冠動脈疾患の定量的評価法として優れた方法であると結論された.

Abstract[Summary] Functional maps of myocardial perfusion and washout rate (WR) were generated from Tl-201 myocardial SPECT study. The functional maps were derived from circumferential profile analyses of tomographic short-axis slices, and the data were plotted using polar display. In order to assess the abnormal area quantitatively, extent and severity maps which correspond to the location and degree of ischemia were generated. Extent and severity scores were also calculated from these maps. This analysis was applied to 51 patients with coronary artery disease who had undergone coronary arteriography. The functional map showed a good correlation with the locations of stenotic coronary arteries, and the three-dimensional pattern of ischemia was well recognized. When the criteria of abnormalities were defined as extent score>0.10 or severity score>5.0, sensitivity for coronary artery disease was 98% (45/46) and specificity was 100% (5/5). The washout rate showed positive correlations with rate-pressure product at peak exercise and also with the number of coronary artery stenosis. In conclusion, these functional mapping and scores were considered to be effective methods for evaluating coronary artery disease.
