Abstract「要旨」 脳血管障害24例に対して123I-IMPによる脳血流SPECTを行い, 133Xe吸入法による局所脳血流像 (rCBF) およびX線CTとの比較を行った. 使用した装置は, リング型検出器を備えた島津社製SET-020で, データ処理装置は日本データジェネラル社製ECLIPSE S-120である. 123I-IMPは1〜3mCiを閉眼にて静注し, 15〜20分後より撮影を開始した. 脳梗塞8例, 脳動脈瘤破裂後血管攣縮6例, 一過性脳虚血発作 (TIA) 3例, モヤモヤ病2例, 合計19例においては, CTと同一の部位に異常所見がみられたのは8例であり, 5例ではCT上の低吸収域より広い範囲に血流低下がみられた. また, 4例 (脳梗塞1例, 破裂脳動脈瘤1例, TIA 2例) では, 123I-IMPにて血流低下領域が描出されたが, CTでは低吸収領域はみられず, 逆に, 2例ではCTにて大脳皮質下に小梗塞巣が描出されたが, 123I-IMPでは異常を認めなかった. このうち, 脳梗塞2例, モヤモヤ病1例を除き, 残り16例に対して133XeによるrCBF検査を併用し, 全例において123I-IMP像とほぼ同様の所見が得られた. 脳動静脈奇型 (AVM) 4例, 高血圧性脳出血1例では, 123I-IMPでは病巣範囲に一致した欠損像がみられたが, 133Xeでは, 深部に病巣が存在したAVM2例, 脳出血の1例で異常は明らかではなかった. 123I-IMPによる脳血流SPECTは, 大脳皮質下の小梗塞巣の検出は困難であるが, 133Xeよりも鮮明な脳血流分布像が得られ, 脳血管障害例における脳血流状態の描出に有用な検査法であると思われた.

Abstract[Summary] Cerebral blood flow imaging with 123I-IMP was clinically compared with 133Xe and X-CT in the patients with cerebrovascular disease (CVD) . The materials consisted of 8 patients with cerebral infarction, 6 patients with angiospasm after ruptured aneurysm, 3 patients with transient ischemic attack (TIA) , 2 patients with moyamoya disease, 4 patients with arteriovenous malformption (AVM) and 1 patient with intracerebral hemorrhage. Instruments used were a circular-detector array emission computed tomograph (SET-020, Simadzu) and a minicomputer system (ECLIPSE S-120, Japan Datageneral) . 123I-IMP images were obtained 15-20 minutes after intra venous injection of 1-3 mCi of 123I-IMP. Among 19 patients with obstructive cerebrovascular disease and ischemic cerebral disease (8 infarction, 6 angiospasm, 3 TIA, 2 moyamoya disease) , 123I-IMP images showed the similar abnormal findings as CT images in 8 patients. In 5 patients, 123I-IMP image showed larger low perfusion area than the low density area shown on CT. In 4 patients without low density area on CT image, a low perfusion area was detected by 123I-IMP and 133Xe. In 2 patients with a small low density area on CT images, 123I-IMP study could not detect the low perfusion area. Findings of 133Xe study were the same as those of 123I-IMP study in 16 patients received both studies. In 4 patients with AVM, 123I-IMP images showed localized low perfusion area in accord with the abnormalities on CT, but 133Xe studies showed high perfusion area in 2 patients, and no abnormality in 2 patients with lesions in the basal ganglia. It is concluded that the 123I-IMP SPECT is a useful study in patient with cerebrovascular disease.
