Abstract | 「要旨」小児肝硬変は胆道閉鎖症, 乳児肝炎, 先天性代謝異常などに合併することが多く, 重篤な合併症の原因となる. RI hepatogramによる肝血流動態の測定法を開発し, 肝硬変の評価に有効性を確認した. 方法は1〜5mCiの99mTcO4-を上肢静脈よりbolus静注し肝右葉正面に設定した関心領域のtime activity curveを作製し, 約1分で出現するpeak値からの減衰速度をpeak値に対する8分値の比率により表示し比較する. 年齢は0歳から18歳の小児肝疾患46例に再検例を含め64回の本検査を行った. 49%から98%までの値をとり, 肝硬変の程度とよく相関し, 肝生検の線維化とも有意に相関した. 本検査値が80%以上の値となると肝硬変の進行を示し, ほとんどの例で (10/11) 消化管出血を併発した. 本法は肝硬変に伴う肝血流動態の変化, ことに静脈系の環流障害をよく反映し, 肝硬変の進行が予想される例に経時的に測定することにより, 消化管出血を予測することができる. |

Abstract | [Summary] RI hepatogram by 99mTcO4- was applied for the evaluation of the change of hepatic circulation in liver cirrhosis of 64 cases of pediatric liver diseases. After bolus intravenous injection of 99mTcO4- , radioactive counts over abdomen were measured and time activity curve of a ROI on the liver right lobe was drawn for ten minutes. The ratio of the count of eight minutes to the peak was calculated. It ranged from 49% to 98% and cases with severe cirrhosis tended to show high ratio. Cases that showed over 80% had very high incidence of intestinal bleeding due to cirrhosis (10/11) . This method indicates the circulatory change of venous phase in the liver after liver cirrhosis, and have practical effects to evaluate the state of liver cirrhosis and estimate intestinal bleeding. |
