Abstract「要旨」左室逆流性弁膜疾患患者14例, 逆流や短絡を有しない各種心疾患患者14例について, SPECTを用いた心拍同期心プールスキャンにより逆流率を求め, X線心臓血管造影のSellers分類と比較した. 対照例では逆流率の平均は5.8±5.9% (n=14) , 左室逆流性弁膜疾患例では平均42.4±15.8% (n=14) であり (P<0.001) , 逆流診断のsensitivityは93% (13/14例) , specificityは100% (14/14例) であった. Sellers II度以下の軽度逆流の4症例の逆流率の平均は24.6±10.1%, III度以上の高度逆流の10症例は49.5±11.0%であり (p<0.005) , 逆流症例中高度逆流を高度と正診するsensitivityは80% (8/10例) , 軽度逆流を軽度と正診するspecificityは75% (3/4例) であった. SPECTを用いた心拍同期心プールスキャンは逆流の有無および重症度の診断にきわめて有用であり, planar法よりも優れていた.

Abstract[Summary] Regurgitant fraction (RF) of 28 cases (14 cases of mitral regurgitation and/or aortic regurgitation, 14 cases without regurgitation) was evaluated by gated cardiac blood pool scan, using single photon emission computed tomography (SPECT) . Data were acquired from 32 views over 180° (LPO to RAO) for end-diastolic (ED) phase and endsystolic (ES) phase. ED and ES short-axis tomogram was reconstructed simultaneously. Ejection count image of short-axis tomogram was obtained by subtraction of ES image from ED image. Atrium was erased in ejection count image because minus count was regarded as zero. LV ejection count (EC) and RVEC were calculated by summing up EC of each slice from apex to base. RF was obtained from formula : (LVEC-RVEC) / LVEC. Calculated RF of 14 control subjects was 5.8+-5.9% (mean+-SD) , RF of 14 cases with regurgitation (Re) was 42.4+-15.8% (p<0.001) . Sensitivity for detecting Re was 93% (13/14 cases) , and specificity was 100% (14/14 cases) . RF of mild Re (I or II : Sellers' classification) was 24.6+-10.1% (n=4) and RF of severe Re (III or IV) was 49.5+-11.0% (n=10) (p<0.005) . Sensitivity for distinguishing severe Re from mild Re was 80% (8/10 cases) , and specificity was 75% (3/4cases) . In planar imaging, RF was calculated by fixed ROI method, RF of 14 control subjects was 39.8+-12.6%, and RF of 14 cases with Re was 60.4+-18.4% (P<0.001) . Sensitivity for detecting Re was 78.6% (11/14 cases) , and specificity was 85.7% (12/14cases) . In conclusion we can evaluate RF of MR and/or AR by gated cardiac blood pool scan using SPECT more accurately than using Planar imaging because we can separate right atrium and ventricle using SPECT.
